SLATE TB testing during February 8-11, 2010, for current school year

The Corporate Employee Health Services Department at Corporate Headquarters is again offering TB testing for the employees in the State Farm Learning and Teaching Exchange (SLATE(tm)) substitute teacher program.

This service is available for those:
  • considering attending a future workshop this winter and spring
  • attended past workshops but didn't get tested at that time
  • had a TB test last year, but now need to get it renewed. (TB tests are good for 12 months)

    All substitutes are required to have a yearly negative TB test. The Regional Offices of Education are responsible for interpreting the state law requirements for how often the test must be re-administered. If we are informed of any additional timing issues- other than a yearly test- we will let you know.

    The dates and hours for administering the TB test are:

    Monday, February 8, 2010 from 9:00 am - 1:00 p.m. - No appointment necessary
    Tuesday, February 9, 2010 from 9:00 am - 1:00 p.m. - No appointment necessary

    A return visit to read the TB test result must be done between 48 and 72 hours after administration. Reading the TB test cannot be done less than 48 hours later nor more than 72 hours later). Employees who come too early will have to come back to Corporate Employee Health Services at the appropriate time for reading the test. Return visit appointments to read the result of the TB test will be given at the time of the testing. At the return visit, the employee will be provided with a form verifying they have had a TB test.

    Return visit appointments for reading the test will be set for either:

    Wednesday, February 10, 2010 from 9:00 am - 1:00 p.m.
    Thursday, February 11, 2010 from 9:00 am - 1:00 p.m.
